You do not have to apply for a new NPI number for your NEW massage therapist employee if they already have an NPI number. You can use the NPI Lookup tool at to find the number if the LMT doesn't know it. The NPI number follows a provider for life and they never get more than one - even if they were an independent contractor previously and are an employee now.
